It turns out there's a vicious cycle the soft porn industry and medical profession have going on. Censorship boards permit only vague genital definition. Too much detail and the image isn't allowed. So the porn industry photoshops nearly all female genitals (that's right boys, nothing you've looked at in a magazine is real) to an unrealistic detail-less "norm". Women are aware only of this "norm" - it's literally what's in the literature and can have image problems if they deviate from those idealized "norm" and may get surgery to "correct" their lady-parts.

So it does appear censorship is driving this genital mutilation.
